Concerto To Bring Maryland Nursing Home Dialysis Population Some Relief
CHICAGO, May 15, 2019 /PRNewswire-PRWeb/ -- Concerto Renal Services (Concerto), one of the nation's leading providers of staff-assisted home hemodialysis services in skilled nursing facilities (SNFs), is acquiring Home Dialysis of Maryland (HDOM). Based in Chicago and serving patients in Illinois, Wisconsin, and Indiana, the acquisition will expand Concerto's annual dialysis population to over 2,200 patients in 40 SNFs. The acquisition is expected to close on June 1, 2019.
Established in 2014 and based in Baltimore, HDOM is a pioneering dialysis provider offering home dialysis and SNF dialysis services throughout Maryland. The acquisition expands Concerto's growth into the East Coast in the developing field of nursing home dialysis.

The acquisition elevates Concerto, a midwestern power in SNF dialysis, to a national stage. As providers and payors move to value-based models, care at home in any setting is increasingly promoted. With approximately 70,000 SNF residents in the U.S. requiring dialysis and this population poised to grow, providing effective in-home dialysis to SNF residents is imperative. Further, the transition to CMS's Patient Driven Payment Model (PDPM) – which rewards higher reimbursement with the provision of nursing care to more clinically complex residents – places a premium on Concerto's model.
