AMGA-TKG Collaborative Reduces Heart Failure Readmission Rates by 12%
NEW YORK, March 7, 2018 /PRNewswire/ -- The Kinetix Group (TKG), a leading life science marketing agency and health system and payer consultancy, partnered with the American Medical Group Association (AMGA) to build the "Best Practices in Managing Patients with Heart Failure (HF) Collaborative," launched by AMGA in 2015. TKG developed the collaborative alongside a global life science company to establish a forum for health systems and medical groups to increase patient self-management and enhance the patient experience of care. It is TKG's second collaborative with the AMGA after the implementation of a successful Rheumatoid Arthritis program launched in 2014.
In a recently published white paper, "Collaborative Learning Leads to Better Care for Heart Failure Patients across the Nation," AMGA announced that among the 15 participating health systems and medical groups, the percentage of heart failure patients readmitted within 30 days decreased from 27% to 15% by the end of the year-long collaborative. The national average is 21.6%.
The AMGA member organizations who participated included some of the nation's foremost providers, such as the Cleveland Clinic, Kelsey-Seybold Clinic, and Summit Medical Group. The interventions they implemented during the program included:
- Developing a heart failure treatment algorithm
- Ensuring patients were prescribed the most effective beta-blocker identified in best practices
- Creating a multidisciplinary team
- Increasing the role of the pharmacist
- Improving transitions of care
- Integrating remote patient monitoring
- Educating patients
